Well-mixed anticoagulated blood is drawn into a graduated tube up to the zero mark, and the tube is seated in the stand where the valve holds it without leaking. After the reading interval set by the laboratory's own procedure, the technician reads the height of the clear plasma layer above the settled red cells directly from the millimetre scale, and records it as the ESR for that sample. With five positions, a small batch of patients or a group of students can be tested side by side.
Accurate readings depend on setup: place the rack on a level, vibration-free bench away from direct sunlight, fans and heat sources, and make sure each tube stands truly vertical, since even a slight tilt speeds up sedimentation. Rinse reusable tubes promptly after use. Like other manual laboratory instruments in India's pathology labs, the ESR rack gives consistent results only when these simple precautions are followed.
